Gear Score in Nightingale is the cumulative average of the rating for all your armor and main-hand weapon. Now, in the early sections, you will have to end up accessing Antiquarian Site of Power which requires you to have a Gear Score of 20.

You can easily get this by crafting new gear while using the Simple Sewing Bench. However, if you want to unlock better gear, you will have to start exploring. The best gear can be unlocked through recipes which can be found from Essence Trades
